St. Paul Radiology can help with select insurance company prior authorization requirements. Prior authorizations for patients with medical insurance from these companies will be electronically submitted on your behalf by St. Paul Radiology when we are provided the following information:
- Patient Name
- Patient Insurance Information- Group, Policy, and ID Numbers
- Patient Date of Birth
- Ordering Clinician Name
- Ordering Clinician Telephone and Fax Numbers
- Imaging Examination Request
- Patient Lab and Imaging History
